Definition

Homozygote Form der Porphyria cutanea tarda mit obligaten Hauterscheinungen.

Ätiopathogenese

Autosomal-rezessiv vererbter Defekt der Uroporphyrinogen-1-Decarboxylase der zu stark verminderter Enzymaktivität führt (5-10%).

Manifestation

Kindheit.

Klinisches Bild

Beginn der ersten Hauterscheinungen in der Kindheit mit phototoxischer Dermatitis innerhalb von Minuten bis Stunden nach der Sonnenexposition. S.a. Porphyria cutanea tarda.

Labor

Erhöhte Konzentrationen von freiem Protoporphyrin in Erythrozyten, Plasma und Stuhl. Prävalenz von Gallensteinen ist erhöht!

Histologie

Wie Porphyria cutanea tarda.

Therapie allgemein

Symptomatisch. Textiler und chemischer Lichtschutz (s.a. Lichtschutzmittel). Engmaschige internistische Kontrolle (Gefahr der protoporphyrininduzierten Leberinsuffizienz).

Interne Therapie

Dauergabe von Karotinoiden (z.B. Carotaben 50–150 mg/Tag).
