Definition

Polyätiologische Pannikulitis bei bestehendem myelodysplastischen Syndrom oder rheumatoider Arthritis; auch als UAW bei Therapie mit BRAF- Inhibitoren, z.B. Vemurafenib,  auftretend. 

Weiterhin wurde das Syndrom bei autoinflammatorischen Syndromen wie dem Sweet-Syndrom und beim Morbus Crohn sowie beim hereditären Alpha-1-Antitrypsin-Mangel beobachtet. 

Manifestation

Vorwiegend bei Frauen mittleren Alters auftretend.

Lokalisation

Am Unterschenkel.

Klinisches Bild

Schmerzhafte Knoten und Platten in der Subkutis mit roter, überlagernder Haut.

Labor

Zeichen der Inflammation mit BSG- und CRP-Erhöhung, Leukozytose und Neutrophilie (evtl. hämatologische Zeichen des myelodysplastischen Syndroms s.dort).  

Histologie

Diffuse lobuläre Fettgewebsnekrose mit basophiler Nekrose, dichten Infiltraten mit neutrophilen Granulozyten, Schaumzellen. Perivaskulär sind auch einige Lymphozyteninfiltrate erkennbar. In der Läppchenperipherie bestehen auch lipomembranöse Fettzellnekrosen und Mikropseudofettzysten.

Therapie

Behandlung der Grunderkrankung. Schnelles Ansprechen auf systemische Glukokortikoide. Symptomatische Lokaltherapie. Reduktion der Therapie mit BRAF-Inhibitoren.
